Need sleep, please tell me your routine

Hi all, please can you tell me your routines that work, my 16wk DS just does not sleep!!! I see every hour at night and cannot do it anymore, I have tried everything I can think of when it comes to some sort of bedtime routine and nothing seems to help, he is worse now then when he was a newborn.
I'm not expecting him to sleep through but need at least 4hours chunk of sleep, surely that isn't asking too much

How much sleep and how many feeds is he getting through the daytime?

I find night times are generally easier if baby is being fed more frequently than you would expect through the daytime and sleeps for longer and more often than you would expect during the daytime.

At 16 weeks I would go for a 2-2.5 hourly EASY routine - this will mean one feed and one sleep in every 2-2.5 hours through the daytime.

E - Eat - Feed marks the start of the routine
A - Awake/Activity - Probably around 60 minutes awake at this age (inc feed0, definitely no more than 90 minutes
S - Sleep - While naps are short and frequent, I have always done bouncy chair and dummy for daytime naps. This allows for bouncing back to sleep whenever baby stirs. I would aim to keep baby asleep until it is 2 hours since the feed, then leave baby to wake naturally and restart the next EASY routine cycle.
Y - You time while baby sleeps.

You can adapt EASY to fit your baby's own body clock as he grows, extending the length of the cycle and the make-up (so more or less time awake or asleep).

EASY also separates feeding and sleeping, which is good for sleep habits long term.
