Please help us undo bad habits!

Just found this on phone- knew it was still here somewhere!

DS is 2.5 and for so long now, one of us has stayed in the room with him until he goes to sleep. When he was in cot we moved futon into room to make life easier for us and now we tend to lay in his bed with him until he drifts off. If hes refused nap that day, this can take about 10 mins. If he's had a nap it can be over an hour.
Then when he wakes in the night he will cry until one of us gets into bed with him and stays there.
I know that its our fault as we've done this for so long and he's bound to panic when were not there on waking. We tried to leave him to go to sleep on his own a few times and it ends with him in such a state that he's sick.
We now also have 1 month dd and DH sometimes has to be away overnight with work.

What's the best way to get him to go to sleep on his own and to spend the whole night on his own, in his own bed after so long with these bad habits we've given him?!
