Letter to boss regarding pregnancy?!

Hi I'm nearly 20 weeks pregnant (first one!) and I need to write a letter to my employer regarding when I'm leaving work, I'm really struggling with what to put and in what format. Do I include when I plan to come back from maternity? Or if I?m wanting to work part time when I go back or when I plan to go back do I include this? (I can do this, as is a nursing home so shifts are flexible. I have looked on the Internet for an exemplar but no luck so far. Any help or advice would be appreciated thanks

you also don't have to request flexible working until after the baby is born so any discussions with your boss prior to that can be just informal

I handed my MATB1 form into HR and just sent them a quick email to confirm my intentions to start maternity leave on 3rd December.
They then wrote back to confirm and also to tell me that I was entitled to a year off, and put in my expected date of return.
You don't need to discuss flexible arrangements now, wait until after babba's born and see how you feel. If you do choose to go back before a year (I'm planning to return in 9 months when SMP runs out) you do need to give them 2 months notice in writing. Again, I'll just send them an email before then to tell them of my intention to return in September and also ask about flexible working.

The only thing you have to tell them formally is that you are pregnant and the date you intent to start your mat leave (of course this may change as time goes on and you see how you feel).

My letter went:

Dear boss

I write to inform you that I am pregnant and my baby is due on 28 Nov. I intent to start my maternity leave on xxxx, and will take 39 weeks maternity leave (or up to 52 weeks if I decide to take unpaid additional mat leave).

You dont need to say how long you are having - they should assume you are having full 52 weeks unless you state otherwise, and then give 2 months notice of your intention to return.

You def shouldnt request part-time at this point, as if they agree they could implement a contractual change which affects your right to return full-time, should you need to (eg change in your financial circs). I just mentioned it informally then requested it formally 2 months before I returned to work.
