What is a Mat B1 Form?

.. and does everyone get one?

.. and what is it used for?

.. and where do I get it?

I'm not currently employed, having taken last year to retrain as a teacher. I know I don't qualify for maternity allowance but do I need this form for my other half to apply for paternity benefit?

Someone mentioned to me that you use it to get free prescriptions etc but I already have an exemption card for this.

I'm normally very up on this, but am currently a bit out of the loop on this one.

Yes ask you midwife for one I think you get it at somethig like 18 weeks, you have to give it to your employer, they need it if your going on maternity leave.

It just shows when your due.

I thought a Mat B1 form was what you gave to your employer as proof of pregnancy, and you get it from your GP, but that could be some other form.

My MatB1 form was given to my employer to prove my due date.

My Dh didn't need it to apply for paternity leave.

I got mine off the MW at my antenatal appts in the hospital (26 Weeks). I think you only get it after 24 weeks+. My DH brought it into work and they photocpied it for his paternity leave entitlement. HTH

My Dh also needed my Mat B1 form to authorise his paternity leave, but that maybe because he got more than the legal minimum? Your DP should ask his HR department, but your MW will give you one at some stage, although you might have to ask for it rather than wait for her to offer.

Thanks all. At my 28 week appointment, the MW asked me if I had my Mat B 1 form. maybe she said, do I have a Mat B1 form. I said no, I didn't qualify for MA. She was probably offering to give me one. D'oh!
