You will hopefully have seen this from other sources, but this is your last chance to take part in a really important survey which looks at parent/carers experiences of personalisation and how they access services, use personal budgets, direct payments etc. Your input could dramatically affect the development of new services across England.

We've moved this to Media Requests now as we don't allow surveys on the main boards unless they're paid for. Sorry if this seems harsh, but we do have to apply the rules fairly to everyone.
