Deed poll, how do I get another copy.

My mother changed my surname when I was a child, a good 17years ago.
Now I've lost the deed poll letter, so how can I obtain a new one? I don't even know the name of the solicitors which she used, and she says she can't remember.
The problem I have is that I have no I.D. whatsoever. I'm trying to apply for a passport and they need to see the original deed poll letter so completely dumbfounded on what to do now!

Also forgot to mention that on my national insurance it has my current surname, and any bills I have, its just my birth certificate that has my former surname.

You could always sign a new change of name deed (please don't refer to it as a deed poll. It won't be.)
