Notice period - requesting specific last day

Can anyone help me word my notice letter?

I work in the NHS and in order to have continual service I need my contract with Trust A to finish on the 4th Dec so I can start with Trust B on the 5th Dec. Both roles are Monday-Friday, so I wouldn't work on the 4th (a Sunday) but need that to be the date on my P45.

You don't need to worry about it. If you work under Agenda for Change, any break in service under 3 months is disregarded for the purposes of determining if you have continuing service. So even if you have a week between the two positions you won't lose out.

Thank you lougle. I have managed, with HR, google and mumsnet, to sort out a resignation letter that works for everyone :) but it is something I shall keep in mind for any future changes in role. I think I got it from the way our Trust deals with working post retirement - they must have 24 hours of retirement before they come back post retirement IYSWIM.
