Ear infection - back to the dr?

We are in Switzerland for 2 months for DH's work and DS has been under the weather and not sleeping pretty much ever since we got here. A couple of weeks ago he developed a fever and we took him to the doctor who diagnosed an ear infection and gave us a 10 day course of antibiotics.

He seemed a lot better whilst taking them, but now three days after finishing the course, he's back to waking every 90 minutes during the night, crying even in his sleep, and clawing at his head. With the exception of the fever these are all the symptoms he had before we took him to the doctor. However, as before, he seems perfectly fine during the day except that he's refusing to nap.

Do you think it's the ear infection coming back but only troubling him at night? Should we go back to the doctor?

I've had chronic ear infections as long as I can remember and it's always worse when lying down. A doctor has explianed why but for the life of me I can't remember, but she suggested putting a pillow under the mattress (for dd who was 6months) as it can help with the pain at night.

Sometimes ear infections need more than one course of antibiotics or a stroger one to clear. I'd go back to the doctor just to make sure.
