I have a very sicky month old baby who possets a lot. Sometimes it's curdled and sometimes it just seems to be milk coming back up- when it's like this it's more forceful than dribble and it feels hot. Is that normal? I think I have an oversupply/fore milk hind milk imbalance that I'm trying to solve but wanted to check if I should worry about the hot sick. He seems well otherwise ..

Could be a bit of reflux but if he seems happy, is gaining weight and feeding well don't worry. What comes up always looks more than it is and it's bound to feel hot, it's been heated up by both of you!
The fore/hind milk issue can be made too much of and messing with feeding can do more harm than good. Just let him finish one side til he comes of then offer the other of he still seems hungry.

What do you notice that makes you think about oversupply / imbalance? What are his nappies like?

Sometimes they're yellow and runny but every other one is green and mucous and smells of vinegar. He struggles at the breast and gulps down milk (often has it leaking from mouth) and is very hiccupy). He also strains and grunts when he poos.

He is gaining weight really well but doesn't seem happy. He arches his back and screams when he comes off my breast - though he could be going through a growth spurt. So hard to know!

We had exactly what you describe, from 4 days to 6 weeks.

The usual bf advice of "you cannot overfeed a bf baby" and "feed at every whimper" made things worse.
I scoured the Web, talked to two lactation consultants, messed about with block feeding and mashed my enormous, sore and engorged boobs in a ridiculous attempt to "mix" fore and hind milk. I asked on here as well. I went to hospital on one wretched day with a bloated, screeching, acid-bottomed baby. Nothing worked until...
a wise old paediatric doctor suggested I introduce a four hourly feeding schedule, using a soother to comfort DS in between feeds. I pumped off some (very watery) milk before each feed but didn't overdo this as I needed to reduce my supply a bit.

Everything was transformed within 24hrs.
I know that the words "schedule" and "bf" are rarely seen in the same sentence but this was a lifesaver for us. I gradually relaxed on the schedule and it went to about every 3 or 3.5 hrs.

Incidentally, we introduced the odd bottle of expressed milk at that time, and I think it was a useful step too.
