Stopping breast feeding

I want to stop BF, my baby is now 12 months, he takes a bottle before bed of formula but is breasted the rest of the day and has 3 meals per day. Should I switch to cows milk exclusively or should I switch to formula? I'm a little confused .. Any help welcome? My baby is v small and at the bottom of the chart so how many feeds per day is correct?
Thanks in advance :-)

You can certainly swap to cows' milk from 12 months. I would start offering it in a beaker if you can (it's better for their teeth). Full fat milk at this age.

Sorry - I can't remember how many feeds per day they need at that age, so will leave that to somebody wiser than I :) (FWIW, my boy is now 24m and has a beaker of milk first thing, then with a tea-time snack and then again at bedtime). We give him full-fat for bedtime, but semi-skimmed for the other two times.
