(assuming, of course, that they ever use insults.... dd didn't at that age but ds does).
Around here it is 'silly patch' to express mild dislike and 'poo poo patch' to express very great antipathy. Goodness knows where they get it from.
Poo face was my dd's favourite along with poo bum and poo breath. Poo anything really.

ds1 used to call me a bossy boiler (from Thomas Tank Engine)
ds3 currently enjoys telling me "Mummy a big man" as he doesn't seem to understand the difference between genders in adults....
Not a toddler but 5yo dd1's current favourite insult is 'Mr Harvey', as in "don't be a silly Mr Harvey" no idea where that came from. My friend's son used to call people a 'pom pom' and roll around laughing.
My 4 yr old has discovered the word bugger and uses it as often as possibe; even to the TA on his first day at school! I'm just ignoring him as much as possible but it's hard when he's shouting it to me in the supermarket!
When DD was little she would call us a "pineapple head" if displeased.

When mine were toddlers (now much more mature bigger) most of the insults revolved around bogies.
Or maybe we just had a lot of colds!

I think they were mainly poo-based here.
Favourites were:

Stinky pants
Oh, and now DS says "Oh Bum Cheeks!" when something goes wrong. Certainly not an expression he got from me.

DS went through a short phase of shouting 'you're painting the house' when v cross.
We eventually realised this was a translation of 'you're a pain in the arse'

Well, DD is 4.5 so not a toddler, but the other day she called me "a fucky poo." 
ds1 3.7 smelling of poo or wee is the only insults he seems to find it hilarious!!! 
DD (3.8) uses 'pomegranate head' or 'you've got a willy' (when insulting me!).
